Before filling out and submitting an application, please double check that you meet or have all of the following qualifications:
ALL three of the items below are required in order for the application to be considered complete and to be considered. If any one of the three is incomplete or incorrect, the application will be denied.

Applications will be processed AFTER March 1, 2019.
Please allow 4-6 weeks from date of receipt for checks to be mailed. April 1: Deadline to INSTALL a PRV April 2-5: Pressure in the system will be increased (incrementally) May 1: Deadline to apply for reimbursement |

The application can be printed prior to filling in. If a printer is not available to you, hard copies of the application are also available in the Town Clerk's Office and the Water Department. Important Notes:

The voucher can be printed prior to filling in. If a printer is not available to you, hard copies of the voucher are also available in the Town Clerk's Office and the Water Department. Important Notes:
Vouchers with reimbursement amounts not matching the paid receipt or in excess of $350 will be adjusted or reduced accordingly.

3. ReceiptProof of payment is required. This must be an ORIGINAL - copies will not be accepted.
If the purchase of the PRV was separate from the installation costs, two original receipts must be included, if you are seeking reimbursement for both the unit and the installation. |

IMPORTANT:
Applications CANNOT be submitted electronically. ONLY original, hard copies of applications, vouchers and receipts will be accepted. PLEASE retain a copy of all documents in your application, especially installation receipts. IF AN APPLICATION IS DENIED FOR ANY REASON, PAPERWORK WILL NOT BE MAILED BACK TO YOU. If an application is denied, you may be eligible to re-submit if you are able to rectify all causes for denial. |
